The Clinical Nurse Specialist/Collaborative Care Manager promotes high quality patient focused cost-effective patient management across the continuum of care for designated high-risk complex patient populations. Responsible for providing and/or coordinating comprehensive care appropriate to the age/population of patients served. Accountable for questioning, investigating, developing, implementing and evaluating cost-effective programs/interventions which promote quality patient outcomes.
